Additionally, we have chosen to be an “out of network provider” for all insurance companies. We strongly believe that you and your doctor are the best decision makers concerning your healthcare. At your request, we will provide you with a “Super Bill” that has the necessary information your insurance carrier needs to process the claim. Your insurance company will be responsible to reimburse you directly in accordance with your insurance policy.
